• Build and maintain trusted, collaborative relationships with assigned providers, including physicians, practice administrators, and staff • Conduct regular virtual and in-person interactions to review performance, co-develop improvement plans, and address barriers to change • Guide providers through Clarify’s change management approach • Serve as the primary point of contact and advocate for assigned providers • Support workflow improvement, education, and awareness of value-based care requirements, quality measures, and performance incentives • Collaborate with internal teams to resolve barriers to provider adoption • Use Clarify’s platform to identify high-opportunity practices, monitor referral patterns, and track behavior change indicators • Translate complex data into clear, actionable narratives for providers and practices • Maintain detailed CRM records in Salesforce, including interactions, change-stage assessments, and engagement activities • Monitor initiatives, leading indicators, and engagement strategy results • Report on territory performance, behavior-change trends, and forecasted impact • Conduct ride-alongs and joint visits with customer liaisons • Assess readiness and skill gaps and develop individualized coaching plans • Design and deliver training on engagement playbooks, insights, change management, and CRM documentation • Monitor customer liaison success metrics and capability development • Improve engagement playbooks and training curricula using field experience and outcomes data
• Bachelor’s degree in Healthcare Administration, Nursing, Public Health, Business, or related field • 5+ years of healthcare experience, including at least 3 years in a direct provider-facing role • Experience coaching, mentoring, or training others in a professional setting • Ability to establish trust and credibility with physicians and practice staff through partnership-oriented engagement • Proficiency interpreting healthcare utilization data, cost trends, referral patterns, and quality metrics • Comfortable working with analytics platforms and Excel • Excellent verbal and written communication skills • Ability to translate complex data into provider-specific narratives and present to clinical and administrative audiences • Experience developing training content, talk tracks, or engagement approaches for field teams • Ability to independently manage a territory, prioritize activities, and drive results with minimal supervision • Proficiency with CRM systems, analytics platforms, and structured engagement tools; Salesforce preferred • Willingness to use AI-powered tools and insights • Player/coach mindset and consultative approach • Comfort with ambiguity and adapting to interpersonal dynamics • Strong work ethic, accountability, and collaborative approach • Preferred: Master’s degree, practice operations or transformation experience, change management or behavioral science exposure, ACO/MSSP/commercial value-based care experience, and familiarity with healthcare analytics platforms • Ideally located within or near assigned market territories; candidates outside priority regions must be willing to travel extensively
