📋 Description

• Facilitate transfers of patients from internal and external facilities into the Cleveland Clinic Health System • Serve as initial point of contact for providers and obtain information needed to initiate patient transfers • Receive transfer requests and gather demographic, insurance, and clinical data for accurate quick registration • Coordinate referrals to appropriate medical services • Connect referring parties with accepting Cleveland Clinic providers to determine transfer appropriateness • Follow up with financial counselors, physicians, case managers, and nurses to expedite transfers • Collaborate with Patient Flow Coordinators and Access RNs to determine appropriate bed availability • Coordinate ambulance transportation within the Cleveland Clinic Health System • Support admission and transfer center initiatives • Maintain 24/7 daily systems to maximize patient flow and optimize enterprise bed capacity • Support centralized communications between referring and accepting provider services • Prepare for and lead bed meetings; monitor and ensure accuracy of electronic patient placement data • Provide backup for patient placement discussions and decisions • Implement HIPPA privacy regulations and Patient Bill of Rights • Oversee emergency and electronic backup methods and procedures supporting patient flow • Provide supervisors feedback on process delays • Meet with Transfer Center team members to discuss problems and develop mitigation strategies • Implement strategic goals related to capacity management, patient flow, and department development • Interpret and support policies, procedures, and safety standards • Develop and promote long-term relationships with referring facilities • Perform other duties as assigned

🎯 Requirements

• High School Diploma or equivalent required • Knowledge of medical terminology required • Minimum 2 years extensive customer service/health care experience, such as patient registration, appointment scheduling or other related experience required • Excellent communication and organizational skills • Ability to handle multiple tasks • Demonstrated independent thinking and problem-solving skills • Strong customer service skills • Working knowledge of computerized hospital admission/bed management systems or related office functions • Sitting for long periods, multitasking, and manual dexterity to operate computers • Normal or corrected vision • Ability to work under stressful and emergency situations while maintaining composure • Ability to perform multiple functions simultaneously without supervision and learn new practices promptly • Must follow standard precautions using personal protective equipment as required • Compliance with annual influenza prevention program • Post-offer controlled-substance testing
