📋 Description

• Assess patients' medical problems and complaints by phone • Obtain patient history and triage information to determine the appropriate care pathway • Assist with management of patient use of the healthcare system • Provide patient education about healthcare needs, community resources, and managed-care responsibilities • Provide telephone advice according to established guidelines and protocols • Arrange provider evaluations through appointments or telephone follow-up • Educate patients and families by phone about minor illnesses, medication usage, health maintenance, blood sugar monitoring, nebulizer usage, prescribed regimens, operational procedures, and specialist referrals • Secure interpreting services when necessary • Participate in Quality Assurance activities • Collaborate with staff to meet HRSA, AAAHC, OSHA, regulatory, and funding requirements • Accurately document patient care in the electronic health record • Provide a safe, comfortable, therapeutic environment for patients and families • Manage patient tasks and patient/provider follow-up according to nurse task guidelines

🎯 Requirements

• Must be bilingual in Spanish and English • Only Colorado residents will be hired for this role • Candidates must live within one hour of our clinics • RN licensure from an accredited school required • Two years of experience in community health preferred • Experience with electronic health records preferred • Sensitivity to low income, ethnic minority community • Critical thinking • Strong computer skills • Ability to flourish in a team management system • Ability to interact with colleagues, team, and all staff in a professional and collegial manner • Required Colorado RN license • Annual influenza (flu) vaccine and tuberculosis screening at hire • Proof of required immunizations before employment begins
