📋 Description

• Conduct virtual patient visits and clinical assessments • Perform triage and identify early warning signs or risk trends • Support chronic disease management and patient education • Monitor telehealth data and escalate concerns appropriately • Help prevent avoidable ER visits and rehospitalizations • Collaborate with interdisciplinary care teams • Document patient interactions accurately and timely • Participate in care planning and performance improvement initiatives • Support development of virtual care best practices

🎯 Requirements

• Active, unencumbered Florida LPN license • Minimum of 2 years of home health experience • Experience with triage and hospitalization prevention • Strong clinical judgment and communication skills • Comfortable using telehealth platforms and EMR systems • High-speed internet connection • HIPAA-compliant, distraction-free workplace • Experience working with high-risk or medically complex patients preferred • Knowledge of chronic disease management preferred • Computer savvy, with ability to use Teams, Outlook, and Excel • Employment contingent upon successful completion of an AHCA Level 2 background screening • Must reside within the state of Florida due to operational, licensing, and/or business requirements
