📋 Description

• Conduct virtual patient visits and clinical assessments • Perform triage and identify early warning signs or risk trends • Support chronic disease management and patient education • Monitor telehealth data and escalate concerns appropriately • Help prevent avoidable ER visits and rehospitalizations • Collaborate with interdisciplinary care teams • Document patient interactions accurately and timely • Participate in care planning and performance improvement initiatives • Support development of virtual care best practices • Provide proactive remote support and clinical intervention to reduce hospitalizations and improve patient outcomes

🎯 Requirements

• Active, unencumbered Florida LPN license • Minimum of 2 years of home health experience • Experience with triage and hospitalization prevention • Strong clinical judgment and communication skills • Comfortable using telehealth platforms and EMR systems • High speed internet connection • HIPPA compliant/distraction free workplace • Computer savvy, with ability to use TEAMS, outlook, excel • Experience working with high-risk or medically complex patients (preferred) • Knowledge of chronic disease management (preferred) • Microsoft Office proficiency (preferred) • Successful completion of an AHCA Level 2 background screening
