The organization brings together providers, caregivers, health plans, advocates, and community organizations to collaborate on enhancing data sharing policies. Their initiative promotes the Data Exchange Framework (DxF), which aims to facilitate secure and efficient exchanges of health and social services information to support better whole-person care.

📋 Description

• Deliver virtual patient care aligned with Oshi's evidence-based care model • Assess, diagnose, and create treatment plans for GI conditions, focusing on DGBIs, GERD, and IBD • Provide care through video visits and chat messaging • Collaborate with GI physicians, dietitians, behavioral health specialists, and care coordinators • Implement coordinated, comprehensive care plans • Follow ROME IV criteria and AGA and ACG guidelines • Document patient interactions accurately and timely in the EHR • Allocate approximately 30 hours per week to direct patient care and 10 hours to documentation, meetings, and administrative responsibilities • Work a flexible schedule including early mornings, evenings, and weekends • Participate in quality improvement, clinical program development, and professional training • Pursue cross-licensure opportunities and maintain good standing with relevant licenses and certifications

🎯 Requirements

• Current licensure as a Certified Registered Nurse Practitioner (CRNP) or Advanced Practice Registered Nurse (APRN) • Master's or Doctoral degree in Nursing • Current certification by ANCC, AANP, or another qualifying agency as an Adult or Family Nurse Practitioner • Minimum of 3 years of experience in gastroenterology specialty in an outpatient setting • Ability to communicate and collaborate effectively with multidisciplinary care providers • Preferred: licensure in 2 additional states desired; candidates must be willing and able to obtain within 60 days of hire • Preferred: experience with or willingness to learn Zoom, Google Suite, Salesforce, Athena, or equivalent • Preferred: prior experience with telehealth platforms • Preferred: comfort operating in an entrepreneurial or start-up environment with evolving workflows and ambiguity
