📋 Description

• Assess the needs of children and families and establish short- and long-term goals • Connect families to housing, employment, social services, and other community resources • Provide mentorship, advocacy, and support • Help families develop life skills, coping skills, healthy relationships, budgeting, parenting, and problem-solving strategies • Create and maintain safety and treatment plans in Case Management Pro • Document sessions, case notes, and reports within required timelines • Monitor and address safety concerns and provide crisis intervention • Document use of evidence-based practices such as Motivational Interviewing • Ensure client understanding and compliance with program expectations and handbook • Maintain communication with clients and team members • Participate in team meetings, trainings, and supervision • Adhere to agency policies, confidentiality standards, and DCS requirements • Deliver services in homes and community settings, with frequent local travel and virtual communication

🎯 Requirements

• Bachelor's or Master's degree in social work, psychology, human services, or a related field preferred • Alternatively, high school diploma, GED, or Associate's degree with at least 4 years of direct casework services experience with children and families • Experience working with children and families preferred • Home-based, child welfare, or community-based experience strongly preferred • Strong verbal and written communication skills • Ability to build rapport and engage families • Crisis management and problem-solving skills • Ability to work independently in community-based settings • Strong organization and time management skills • Ability to collaborate with internal and external stakeholders • Understanding of trauma-informed and strength-based approaches • At least 21 years of age • Valid driver's license, reliable transportation, and current auto insurance • Ability to pass background checks and required clearances • CPR and First Aid certification, or ability to obtain • Ability to work a flexible schedule, including evenings and weekends • Must be trained and competent in required evidence-based practices
