📋 Description
• Provide in-person and telephonic Medical Case Management • Utilize medical and nursing knowledge to discuss treatment plans • Conduct assessments, planning, implementation, and evaluation of patient’s progress • Evaluate treatment plans for appropriateness, medical necessity, and cost effectiveness • Attend doctors, home visits, and discharge planning conferences • Implement care such as delivering durable medical equipment and nursing services
🎯 Requirements
• Experience as an RN Medical Case Manager is ideal • Clinical background in orthopedics, neurology, or rehabilitation preferred • Graduate of accredited school of nursing • Current RN Licensure in state of operation • Certification as a CCM, CIRS, or other Case Management certifications preferred • Valid driver's license and reliable transportation required for local travel
