📋 Description

• Manage two CDC-funded projects in partnership with the Association of State and Territorial Health Officials • Serve as project facilitator and project manager for both projects • Guide project planning and implementation, coordinate project teams, develop partner relationships, and manage day-to-day operations • Participate in project-specific, CAHC-, NACDD-, and CDC-required meetings • Manage contractual processes and ensure subject matter experts and vendors fulfill scopes of work • Manage budgets, contract submissions, and project-related expenditures • Coordinate jurisdictional relationships and services in the Pacific and Caribbean • Coordinate program planning, implementation, and evaluation/reporting with ASTHO and CDC • Align and coordinate NACDD staff, subject matter experts, and vendors across projects • Report project progress through meetings, reports, and communications • Create timelines and ensure completion of workplan activities • Facilitate project meetings and trainings, including agendas, meeting leadership, summaries, and follow-up • Identify cross-program collaboration and resource-sharing opportunities • Measure and communicate project impact • Collaborate on evaluation and communications activities • Support other Diabetes Portfolio, CAHC, and NACDD programs as needed

🎯 Requirements

• Bachelor's degree in Biology, Sociology, Public Health or other health-related fields • Five (5) to seven (7) years of relevant experience in planning, implementing/leading, and convening chronic disease prevention and/or management or health promotion programs • Experience working with CDC or other federal agencies on programs related to state-level chronic disease prevention and health promotion • Experience working with U.S. Territories and Freely Associated States (T/FAS) on projects or initiatives • Experience coordinating and delivering context-specific meetings, training, and/or technical assistance • Experience managing work plans, particularly for grants/cooperative agreements • Experience developing tools and resources, facilitating meetings and peer-to-peer learning opportunities, and giving presentations • Strong project management, prioritization, and planning skills • Excellent multitasking, problem-solving, budget management, interpersonal, organizational, oral, and written communication skills • Experience with grant writing and management • High proficiency with Microsoft Office programs, including Word, Excel, and PowerPoint • High proficiency with virtual meeting platforms such as Zoom or Teams and project management platforms such as Smartsheet • Must be available for meetings between 5–9pm EST, Monday–Thursday, and accommodate time zones across the U.S. Territories and Freely Associated States • Available to travel as needed • Master’s degree in Public Health, Public Health Administration, or related field preferred • Prior association or nonprofit experience preferred • Prior state, county, or local health department experience preferred • Prior diabetes prevention or diabetes self-management program experience preferred • Spoken and written proficiency in languages spoken in the U.S. Territories and Freely Associated States advantageous but not required • Cover letter required
