📋 Description

• Develop patient coaching relationships supporting care plans and quality of life • Provide empathic support to patients and families dealing with chronic conditions • Communicate abnormal findings and effectively report findings to the cardiology care team • Monitor and encourage adherence to care plans and participation • Support new patients through the enrollment process • Liaise between patients, practices, and providers • Identify and escalate patient changes to providers to reduce hospitalizations and urgent care visits • Work with the EMR and CCM platform • Provide basic monitor troubleshooting for patients using the CCM platform • Participate in continued team education through shared learnings • Contribute to high-quality service for physicians and patients • Stay current with and comply with applicable regulations and company policies

🎯 Requirements

• High School diploma or GED • 1 year of experience in remote chronic care management • Experience with cardiology patients, specifically heart failure • Registered Nurse (RN), Medical Assistant (MA), or Certified Nurse Assistant (CNA) • Experience with EMRs, specifically Athena, NexGen, GEMMS, and eClinicalWorks • Great communication and multi-tasking skills • Ability to work 8-hour shifts based on customer/patient time zone • Ability to follow clinical protocols • Compliance with applicable state and federal regulations and company policies
