📋 Description

• Assess, plan, implement, and coordinate member care telephonically and/or virtually • Develop individualized care plans and wellness strategies • Identify clinical and social barriers to health outcomes • Connect members with programs, benefits, and community resources • Apply clinical judgment to manage complex health conditions and reduce risk factors • Engage members using communication and motivational interviewing techniques • Work in a queue-like setting • Partner with members, providers, and care teams • Attend meetings and training as needed, with less than 5% travel • Provide occasional on-call weekend and holiday coverage on a volunteer basis

🎯 Requirements

• Must live in California • Active, unrestricted RN license in state of residence • Willingness to obtain additional state licenses • 3+ years of acute care RN experience (medical, surgical, specialty, ICU, pediatrics, discharge planning, or case management) • Associate Degree in nursing • Comfortable working with multiple computer applications and monitors • Direct/hardwired internet connection to modem/router within 7 feet of computer • Minimum internet speed of 25 Mbps download and 3 Mbps upload • WiFi and satellite internet are not permitted • Quiet, secure, private designated home workspace compliant with CVS Health and HIPAA guidelines • Ability to work within the state and city where currently living • Ability to provide own workspace furniture • Comfortable using multiple monitors and applications simultaneously • Ability to use digital communication channels including email, calendar invites, Teams messaging, and virtual meetings • Experience with Microsoft Office 365 applications or similar Google Workspace tools • Ability to access secure systems such as VPN and EHR portals • Ability to follow computer security practices, manage strong passwords, and recognize suspicious emails or links • Ability to troubleshoot common technical issues • Preferred: Compact RN license • Preferred: BSN • Preferred: 1+ year of case management experience • Preferred: Certified Case Manager (CCM) certification • Preferred: Strong customer service and member engagement skills • Preferred: Excellent phone-based communication skills • Preferred: Candidates living in MST or PST
