📋 Description

• Conduct telephonic engagement with members and coordinate their care for 50–75% of the day • Compile clinical information and develop individualized care plans with members • Provide evidence-based disease management education and support • Involve appropriate members of the interdisciplinary care team • Provide care coordination for a seamless healthcare experience • Document care management activity in members’ electronic health records • Collaborate with the Interdisciplinary Care Team to address barriers to care and maintain stable health • Connect members with health plan benefits and community resources • Meet regulatory requirements within specified timelines • Support Care Team members through clinical decision-making and guidance • Participate in special projects, process improvement, and mentoring as assigned

🎯 Requirements

• Active and unrestricted Compact Registered Nurse (RN) licensure in the state of residence • Proficient in Microsoft Office Suite, including Word, Excel, Outlook, OneNote, and Teams • Access to a private, dedicated space to conduct work effectively • Dependents must have separate care arrangements during work hours • Confidence working at home and independently using virtual collaboration tools • 3+ years of nursing experience • 2+ years of case management, discharge planning and/or home healthcare coordination experience • Associate’s of Science in Nursing (ASN) Degree and relevant experience in a health care-related field (REQUIRED) • Experience or willingness to learn care management within Medicare and Medicaid managed care • Familiarity with community resources and services • Ability to use healthcare technology tools • Effective verbal and written communication • Excellent customer service and engagement skills
