📋 Description

• Manage enrollment for the Children’s Specialty program and child and family welfare population health members • Manage a low-tier member caseload • Conduct initial member review and triage • Develop Wellness Plans and provide community resources • Review gaps in care and administer health questionnaires • Complete outbound calls to identify and engage community resources • Screen members to identify needed medical services, make referrals, and coordinate services • Identify triggers for referral to Case Management, Disease Management, Mixed Services, and Specialty Programs • Use Aetna systems to build, research, and enter member information • Coordinate healthcare service delivery under nurse or medical director direction • Promote internal and external communication with healthcare providers and team members • Conduct non-medical research for open-case management • Support team members by answering calls, researching information, and solving problems • Maintain accurate documentation and protect member confidentiality • Assist with research and resolution of claims payment issues • Complete detailed electronic-record documentation for every member call • Meet daily metrics, deliverables, and deadlines • Participate in multidisciplinary teams and conduct persistent outreach

🎯 Requirements

• Must reside in Ohio • 2–4 years’ experience in healthcare field or working with foster, child and family welfare populations • Effective communication, telephonic and organizational skills • Ability to manage multiple priorities and adapt to change • Strong customer service skills • 2+ years’ demonstrated proficiency with personal computers, keyboard, multi-system navigation, and MS Office Suite applications (Teams, Outlook, Word, Excel, etc.) • Reliable transportation and willingness to travel in-state as needed • Some in-state travel (5–10%) may be required • Must pass CANS certification exam within the first 3 months of employment • Motivational interviewing skills preferred • Call Center experience preferred • Managed Care experience preferred • High school diploma or G.E.D.
