📋 Description

• Utilize critical thinking and judgment to collaborate and inform the case management process • Facilitate appropriate healthcare outcomes for members • Assist with appointment scheduling • Identify and assist members with accessing benefits • Provide education to members using care management tools and resources • Collaborate with members, providers, and community organizations • Address members' healthcare and social determinant needs • Travel up to 75% from the home location throughout Metro East/Southern Illinois and surrounding areas

🎯 Requirements

• Must reside in the state of Illinois • Must possess reliable transportation and be willing and able to travel up to 75% of the time from home location in and around Metro East/Southern Illinois • Minimum 3-5 years clinical practical experience • Confidence working at home/independent thinker, using tools to collaborate and connect with teams virtually • Excellent analytical and problem-solving skills • Effective communications, organizational, and interpersonal skills • Ability to work independently • Effective computer skills including navigating multiple systems and keyboarding • Proficiency with MS Word, Excel, Outlook, PowerPoint, and proprietary applications • Associate's degree required • Active and unencumbered Registered Nurse license in the state of Illinois • Direct/hardwired internet connection to a modem/router within 7 feet of the computer • Minimum internet speed of 25 Mbps download and 3 Mbps upload • Quiet, secure, private designated home virtual work location compliant with CVS Health and HIPAA guidelines • Must work within the state and city where currently residing • Must provide workspace furniture • Experience setting up and using multiple monitors and navigating multiple applications • Experience with Microsoft Office 365 applications or similar Google Workspace tools • Ability to access secure systems such as VPN and EHR portals • Ability to manage strong passwords and recognize suspicious emails or links • Ability to resolve common technical issues independently • Openness to learning new skills as the workplace evolves
