📋 Description

• Assess, plan, implement, and coordinate care for members telephonically and/or virtually • Develop individualized care plans and wellness strategies • Identify clinical and social barriers to health outcomes • Connect members with programs, benefits, and community resources • Apply clinical judgment to manage complex health conditions and reduce risk factors • Engage members through communication and motivational interviewing techniques • Work in a queue-like setting • Partner with members, providers, and care teams to coordinate care and improve patient outcomes • Work Monday-Friday, 9:00 AM-5:30 PM EST • Participate in occasional on-call weekends and holidays on a volunteer basis • Travel less than 5% for meetings, training, or licensure requirements

🎯 Requirements

• Must live in California • Active, unrestricted RN license in your state of residence • Willingness to obtain additional state licenses • 3+ years of acute care RN experience • Comfort working with multiple computer applications and monitors • Associate Degree in nursing • Direct/hardwired internet connection to a modem/router within 7 feet of the computer • Minimum internet speed of 25 Mbps download and 3 Mbps upload • WiFi and satellite internet are not permitted • Quiet, secure, private designated home virtual work location compliant with CVS Health and HIPAA guidelines • Ability to work within the state and city where currently living • Ability to use multiple monitors and applications simultaneously • Ability to communicate via email, calendar invites, Teams messaging, and virtual meetings • Experience with Microsoft Office 365 applications or similar Google Workspace applications • Ability to log in to secure systems such as VPN and EHR portals • Ability to follow computer security practices, manage strong passwords, and recognize suspicious emails or links • Ability to troubleshoot common technical issues independently • Preferred: Compact RN license • Preferred: BSN • Preferred: 1+ year of case management experience • Preferred: Certified Case Manager (CCM) certification • Preferred: Strong customer service and member engagement skills • Preferred: Excellent phone-based communication skills • Prefer candidates living in MST or PST
