📋 Description

• Collaborate and inform the case management process using critical thinking and judgment • Facilitate appropriate healthcare outcomes for members • Assist with appointment scheduling • Identify and assist members with accessing benefits and education • Use care management tools and resources • Serve dual-eligible Medicare and Medicaid members with complex health and social challenges • Collaborate with members, providers, and community organizations • Address members’ healthcare and social determinant needs • Work from home while traveling up to 50-75% of the time across Will, Kankakee, and surrounding areas

🎯 Requirements

• Must reside in the state of Illinois • Must live in or around Will/Kankakee Counties in Illinois • Reliable transportation and willingness and ability to travel up to 50-75% of the time from the home location in and around Will, Kankakee and surrounding areas, IL • Minimum 3-5 years clinical practical experience • Confidence working at home/independent thinker, using tools to collaborate and connect with teams virtually • Excellent analytical and problem-solving skills • Effective communication, organizational, and interpersonal skills • Ability to work independently • Effective computer skills, including navigating multiple systems and keyboarding • Proficiency with standard corporate software applications, including MS Word, Excel, Outlook, and PowerPoint, as well as proprietary applications • Associate's degree required • Active and unencumbered Registered Nurse license in the state of Illinois • Direct/hardwired internet connection to a modem/router within 7 feet of the computer • Minimum internet speed of 25 Mbps download and 3 Mbps upload • Quiet, secure, private designated home work location compliant with CVS Health and HIPAA guidelines • Must work within the state and city where currently residing • Microsoft Office 365, Teams, Outlook, Word, Excel, and PowerPoint, or similar Google Workspace experience • Ability to access secure systems such as VPN and EHR portals • Ability to manage strong passwords and identify suspicious emails or links • Ability to troubleshoot common technical issues • Preferred: 2-3 years Care Management, discharge planning and/or home health care coordination experience • Preferred: Certified Case Manager • Preferred: Bilingual
