📋 Description

• Provide remote patient care and support through telehealth platforms • Conduct virtual patient assessments, including reviewing medical histories and current symptoms • Accurately assess and document patient conditions, histories, and care interactions • Educate patients and their families on medical conditions, medication adherence, and self-care practices • Work closely with healthcare providers to develop and implement individualized care plans • Maintain accurate records and ensure adherence to healthcare regulations and privacy laws • Stay current with medical advancements, telehealth protocols, and best practices through continuing education • Provide compassionate, professional, and effective communication during patient interactions • Support the FuzeRx Central Operations telehealth care team through remote outreach to Medicare members • Ensure member compliance with a new Stars measure and confirm successful emergency department discharge • Close other HEDIS measures as needed

🎯 Requirements

• Bachelor of Science in Nursing (BSN) degree • Active, non-restricted compact state nursing license with primary state of licensure in an eligible compact state • 2+ years of experience as a Registered Nurse • 1–2 years of experience in a remote healthcare or telehealth setting • Strong computer skills and familiarity with electronic medical record (EMR) systems • Ability to communicate clearly and effectively in both verbal and written forms • Demonstrated ability to follow standard operating procedures and adapt in a fast-paced environment • Strong customer service and interpersonal skills, especially in high-volume or high-stress scenarios • Ability to work independently and as part of a collaborative care team • Experience working in a startup environment (preferred) • Ability to work at a computer terminal for extended periods and perform specified physical tasks, including lifting, carrying, pushing, pulling, and moving objects up to 20 pounds • Applicants must be authorized to work for any employer in the U.S.
