📋 Description

• Collaborate and inform the case management process using critical thinking and judgment • Facilitate appropriate healthcare outcomes for members • Assist with appointment scheduling • Identify and assist members with accessing benefits and education • Use care management tools and resources • Serve dual eligible Medicare and Medicaid populations with complex health and social challenges • Collaborate with members, providers, and community organizations • Address members’ healthcare and social determinant needs • Travel up to 50–75% of the time from the home location in and around Cook County and surrounding areas • Work independently from a designated home workspace and collaborate with teams virtually • Navigate multiple systems and applications, including secure VPN and EHR portals • Communicate through email, calendar invites, Teams messaging, and virtual meetings • Resolve common technical issues and contact IT when needed

🎯 Requirements

• Must reside in the state of Illinois • Must live in or near Cook County and surrounding areas in Illinois • Must possess reliable transportation and be willing and able to travel up to 50–75% of the time from home location in and around Cook County and surrounding areas in Illinois • Minimum 3–5 years clinical practical experience • Confidence working at home/independent thinker, using tools to collaborate and connect with teams virtually • Excellent analytical and problem-solving skills • Effective communication, organizational, and interpersonal skills • Ability to work independently • Effective computer skills including navigating multiple systems and keyboarding • Proficiency with standard corporate software applications, including MS Word, Excel, Outlook, and PowerPoint, as well as proprietary applications • Associate's degree required • Active and unencumbered Registered Nurse license in the state of Illinois • Direct/hardwired internet connection to a modem/router within 7 feet of the computer, with minimum 25 Mbps download and 3 Mbps upload; WiFi and satellite internet are not permitted • Quiet, secure, private designated home virtual work location compliant with CVS Health and HIPAA guidelines • Must work within the state and city where currently living • Must provide own workspace furniture • Must obtain required internet speeds and adhere to Work From Home requirements • Comfortable setting up and using multiple monitors and navigating multiple applications simultaneously • Ability to communicate via email, calendar invites, Teams messaging, and virtual meetings • Experience with Microsoft Office 365 applications or similar Google Workspace tools • Ability to log in to secure systems such as VPN and EHR portals, lock screens, manage strong passwords, and recognize suspicious emails or links • Ability to resolve common technical issues independently and contact IT for unresolved issues
