📋 Description

• Develop proactive plans of care addressing identified issues and enhancing short- and long-term outcomes • Assess members' needs and benefits using clinical tools and information/data review • Apply clinical judgment to reduce risk factors and barriers and address complex health and social indicators • Conduct holistic assessments using claims and information from various sources • Assess needs for referrals to clinical resources and interdisciplinary team members • Collaborate with supervisors, healthcare stakeholders, members, providers, and community organizations • Present cases at interdisciplinary case conferences • Coordinate case management activities and evaluate members' medical needs • Use motivational interviewing to maximize member engagement and understand health needs • Travel 25-50% within Monmouth/Ocean Counties to meet members face to face

🎯 Requirements

• Minimum 3+ years of clinical practice experience • Active and unrestricted RN licensure in New Jersey required • Willing and able to travel 25-50% of the time using own vehicle • Reliable transportation required • Must reside close to or within Monmouth/Ocean Counties, New Jersey • Certified Case Manager preferred • Minimum 2+ years Care Management, Discharge Planning and/or Home Health Care Coordination experience preferred • Dedicated workspace free of interruptions • Separate dependent care arrangements during work hours • Associate's Degree required • Bachelor's degree preferred • Confidence working at home/independent thinker • Excellent analytical and problem-solving skills • Effective communications, organizational, and interpersonal skills • Ability to work independently • Effective computer skills, including navigating multiple systems and keyboarding • Proficiency with MS Word, Excel, Outlook, PowerPoint, and proprietary applications • Bilingual preferred
