📋 Description

• Administer the care coordination plan to assess patient needs and ensure seamless transitions between care settings • Analyze complex patient data from medical history, diagnostic test results, and treatment plans • Apply case management and nursing knowledge to organize patient files • Communicate through internal platforms to exchange messages, conduct video conferences, share files, and collaborate on patient care plans • Conduct routine utilization reviews to ensure access to appropriate cost-effective care • Configure the case management system for disease management and utilization review • Track patient progress and manage specific conditions • Coordinate analytics projects and generate reports on key performance health indicators • Design processes for safe and timely discharge transitions from hospital to home • Develop resource management using community resources • Advocate for patients and coordinate services across healthcare providers

🎯 Requirements

• 5+ years of relevant work experience • Active and unrestricted Registered Nurse Licensure in state of residence • Working knowledge of problem solving and decision making skills • Working knowledge of medical terminology • Working knowledge of digital literacy skills • Ability to deal tactfully with customers and community • Ability to handle sensitive information ethically and responsibly • Ability to consider the relative costs and benefits of potential actions to choose the most appropriate option • Ability to function in clinical setting with diverse cultural dynamics of clinical staff and patients • Associate’s Degree in Nursing (REQUIRED) • Direct/hardwired internet connection to a modem/router within 7 feet of the computer • Minimum internet speed of 25 Mbps download and 3 Mbps upload • WiFi and satellite internet are not permitted • Quiet, secure and private designated home virtual work location compliant with CVS Health and HIPAA guidelines • Must work within the state and city where currently living • Must provide own workspace furniture • Must obtain required internet speeds and adhere to Work From Home requirements if hired
