📋 Description

• Use a collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy • Meet individuals’ and families’ comprehensive health needs through communication and available resources • Promote quality and cost-effective outcomes for Medicare members • Perform in-person case management activities within a designated geographic area when directed by leadership or business needs • Work Monday through Friday, 8:00 AM to 5:00 PM Eastern

🎯 Requirements

• Active and unrestricted Compact license • Minimum 3-5 years clinical practical experience preferred, including diabetes, CHF, CKD, post-acute care, hospice, palliative care, or cardiac experience with Medicare members • Minimum 2-3 years CM, discharge planning and/or home health care coordination experience • Registered Nurse with active state license in good standing within the region where job duties are performed • Bachelor’s degree or equivalent experience; required to obtain a bachelor’s degree within 3-5 years as part of role development • Ability to occasionally travel within a designated geographic area for in-person case management activities • Bilingual preferred • Excellent analytical and problem-solving skills • Effective communication, organizational, and interpersonal skills • Ability to work independently, including working from home • Proficiency with MS Word, Excel, Outlook, PowerPoint, Microsoft Office 365, Teams, or similar Google Workspace applications • Efficient computer skills, including navigating multiple systems and keyboarding • Willingness and ability to obtain multistate RN licenses if needed; company will provide • Direct/hardwired internet connection to a modem/router within 7 feet of the computer • Minimum internet speed of 25 Mbps download and 3 Mbps upload • Quiet, secure, private designated home workspace compliant with CVS Health and HIPAA guidelines • Ability to set up and use multiple monitors and navigate multiple applications simultaneously • Ability to communicate via email, calendar invites, Teams messaging, and virtual meetings • Ability to log in to secure systems such as VPN and EHR portals, lock screens, manage strong passwords, and recognize suspicious emails or links • Ability to resolve common technical issues independently • Openness to learning new skills as the workplace evolves
