📋 Description

• Provide in-home health evaluations for health plan members • Conduct approximately 45-minute visits involving one-on-one health conversations • Review medications and medical history • Perform physical evaluations • Perform additional diagnostic tests when ordered by the health plan, including diabetic eye exams and spirometry • Use an iPad and clinical workflow for each evaluation • Visit people nationwide and connect them to appropriate care • Provide Signify Health services, including Diagnostic & Preventive Services such as spirometry, diabetic eye exams, and bone density screening • Conduct virtual visits as needed • Travel locally and within licensed states as needed • Build trusted relationships and improve health outcomes

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Board certification (required) • Multi-state licenses (preferred) • Ability to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, and airborne allergens or irritants • Ability to travel locally and within licensed states as needed • Ability to conduct virtual visits as needed • Nurse Practitioner or Physician Assistant qualification
