📋 Description

• Provide in-home health evaluations for health plan members • Conduct approximately 45-minute visits, including medication and medical history review and physical evaluation • Perform additional diagnostic tests when ordered by the member’s health plan, including diabetic eye exams and spirometry • Build trusted relationships and help improve health outcomes • Visit people nationwide and connect them with appropriate care • Travel locally and within licensed states as needed • Conduct virtual visits as needed • Provide Signify Health services, including Diagnostic & Preventive Services such as spirometry, diabetic eye exams, and bone density screening • Use an iPad and clinical workflow during evaluations • Do not prescribe medicine, order lab tests, or alter current treatment regimens

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Board certification (required) • Multi-state licenses (preferred) • Ability to travel locally and within licensed states, as needed • Ability to conduct visits in private residences that may contain household pets, secondhand smoke, and other airborne allergens or irritants • Ability to use an iPad and clinical workflow for evaluations • Nurse Practitioner or Physician Assistant qualification
