📋 Description

• Provide In-Home Health Evaluations in members’ homes • Engage with people and help bridge gaps in care • Travel primarily across a defined coverage area, potentially up to 100% of the time • Conduct approximately 45-minute visits • Review medication and medical history • Perform physical evaluations • Provide additional diagnostic tests when ordered, including diabetic eye exams, spirometry and bone density screening • Use an iPad and clinical workflow for each evaluation • Conduct virtual visits as needed • Provide Diagnostic & Preventive Services products • Build trusted relationships and help improve health outcomes • Visit people nationwide and connect them to appropriate care • Travel locally and within licensed states, including possible overnight hotel stays, car rentals and flights

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Board certification (required) • Able to obtain and maintain multi-state licenses in coverage area, as required • Able to cover all counties within state and flex to other areas of business need • Must be able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants • Nurse Practitioner or Physician Assistant qualification • Willingness and ability to travel locally and within licensed states, potentially up to 100% of the time • Ability to conduct virtual visits as needed • Ability to use an iPad and clinical workflow • Ability to perform medication and medical history reviews, physical evaluations, and diagnostic tests as ordered
