📋 Description

• Provide In-Home Health Evaluations in members’ private residences • Travel across a defined coverage area, potentially requiring overnight hotel stays, car rentals, and/or flights • Build trusted relationships and help improve health outcomes • Conduct approximately 45-minute visits • Review medications and medical histories • Perform physical evaluations • Perform additional diagnostic tests when ordered, including diabetic eye exams, spirometry, and bone density screening • Use an iPad and clinical workflow for each evaluation • Answer health questions and provide health education • Conduct virtual visits as needed • Provide Signify Health services, including Diagnostic & Preventive Services products • Cover all counties within the state and flex to other areas based on business needs

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Board certification (required) • Able to obtain and maintain multi-state licenses in coverage area, as required • Able to cover all counties within state and flex to other areas of business need • Comfortable and able to safely perform duties in homes containing household pets, secondhand smoke, and potential airborne allergens or irritants • Nurse Practitioner or Physician Assistant qualification • Ability to travel locally and within licensed states • Ability to travel up to 100% of the time • Ability to conduct virtual visits as needed • Ability to use an iPad and clinical workflow • Ability to work 40 hours per week
