📋 Description

• Conduct comprehensive assessments of members' behavioral health, physical health, and psychosocial needs • Develop individualized care plans promoting improved health outcomes and well-being • Collaborate with providers, care teams, and community resources to coordinate appropriate care • Apply clinical judgment to identify and address risk factors, barriers to care, and complex health needs • Use Motivational Interviewing techniques to engage members and encourage long-term behavior change • Provide crisis intervention and ensure timely access to treatment and support services • Coordinate behavioral health and substance use disorder services across the continuum of care • Use technology and clinical systems to document care and manage member interactions

🎯 Requirements

• Master's Degree in Social Work, Counseling, or a related Behavioral Health field • Active, unrestricted independent Behavioral Health license (LCSW, LISW, LMSW, LCPC, LPC, LMFT, LMHC, or equivalent) • 3+ years of post-master's direct clinical experience in hospital, ambulatory, outpatient, or similar behavioral health settings • 3+ years of experience treating mental health and substance use disorders • 3+ years of experience in crisis intervention and using Motivational Interviewing techniques • Strong technical proficiency and ability to document while engaging members telephonically • Direct/hardwired internet connection to a modem/router within 7 feet of the computer • Minimum internet speed of 25 Mbps download and 3 Mbps upload • Quiet, secure, private designated home work location compliant with CVS Health and HIPAA guidelines • Must work within the state and city where currently living • Must provide own workspace furniture, such as a desk or standing desk • Ability to use multiple monitors and navigate multiple applications simultaneously • Ability to communicate via email, calendar invites, Teams messaging, and virtual meetings • Experience with Microsoft Office 365 applications or similar Google Workspace tools • Ability to log in to secure systems such as VPN and EHR portals • Ability to follow computer, password, and information-security practices • Ability to troubleshoot common technical issues and contact IT when needed • Preferred: Managed Care or Utilization Review experience • Preferred: Case Management experience • Preferred: Discharge Planning experience • Preferred: Experience working with complex populations and multidisciplinary teams
