📋 Description

• Conduct comprehensive evaluations of members’ health status and risks using clinical tools and information/data review • Establish plans of care based on members’ health status, risks, education, and support needs • Identify quality-of-care issues through established channels • Use influencing and motivational interviewing skills to maximize member engagement and promote lifestyle and behavior changes • Provide coaching and support using interviewing and active listening skills to empower members to make independent healthy lifestyle choices • Develop and monitor plans of care in collaboration with members and/or attending physicians • Follow internal policies and procedures in compliance with regulatory and accreditation guidelines • Engage, educate, and promote or influence members in decisions related to achieving and maintaining optimal health • Conduct appointments via telehealth or virtual channels

🎯 Requirements

• Must have CDCES (Certified Diabetes Care and Education Specialist) • Certified Diabetic Educator • Registered Dietitian with licensure • 3+ years of coaching experience required • Bachelor’s degree in a health-related field, such as Dietetics, Health Education, Wellness, Health Promotion, or equivalent experience • Experience in wellness-related areas such as diabetes, weight management, stress management, nutrition, physical activity, or healthy lifestyle behavior change • Ability to communicate, problem solve, and work effectively with people • Good judgment in challenging situations • Health Coach Certification or Fitness certification recommended • Background in motivational interviewing, active listening, Transtheoretical Model, mindfulness-based, and other evidence-based coaching techniques preferred • Excellent computer skills, including Microsoft Office applications such as Excel, Word, PowerPoint, and Outlook • Ability to build relationships with wellness champions and build a culture of health • Availability to work Monday through Friday, including one late night until 9 p.m. EST each week
