📋 Description

• Provide comprehensive diabetes, hypertension, hyperlipidemia, and weight loss care, education, and support through chat, phone, and video calls following internal protocols • Create thorough consults aligned with internal protocols for physicians and the provider team • Collaborate with Patient Care Advocates to ensure member success • Develop tailored nutrition and lifestyle plans based on members’ medical conditions, goals, and preferences • Collaborate with pharmacists and provide medication-related care and guidance • Track member progress through regular check-ins, assess intervention effectiveness, and adjust plans • Provide motivational interviewing, coaching, and counseling regarding diet, exercise, and stress management • Use member data and health metrics to inform care recommendations and help interpret lab results • Maintain accurate member documentation in accordance with internal and legal standards • Work 2 weekends per quarter

🎯 Requirements

• Licensed Registered Dietitian Nutritionist • Certified Diabetes Care and Education Specialist (CDCES), Board Certified-Advanced Diabetes Management (BC-ADM), or eligible for certification • Strong knowledge and experience providing care for diabetes, hypertension, hyperlipidemia, and obesity • Minimum 5 years of direct patient-care experience, preferably in a clinical or telehealth setting • Experience in medication adjustment and using an EMR • Ability to build trusting patient relationships through compassionate, tailored communication • Experience collaborating within an interdisciplinary healthcare team • Experience providing care through virtual platforms, including chat, phone, and video • Comfort using digital health tools and patient-management systems • Quiet, dedicated workspace supporting patient privacy and confidentiality • Reliable high-speed internet connection • Flexible and resilient in a changing startup environment • Prior digital-health experience focused on chronic conditions preferred • Multilingual capabilities, particularly Spanish, preferred
