📋 Description

• Assess, plan, implement, and coordinate case management activities for members • Develop proactive plans of care to address identified issues and improve short- and long-term outcomes • Use clinical tools and data review to evaluate member needs and benefits • Apply clinical judgment to reduce risk factors and address complex health and social indicators • Conduct assessments using claims and other information sources, including co-morbidities and multiple diagnoses • Assess need for referrals to clinical resources and interdisciplinary team members • Collaborate with supervisors and healthcare stakeholders to overcome barriers and meet goals • Present cases at interdisciplinary case conferences • Follow case management processes, regulatory requirements, and company policies • Use motivational interviewing to engage members and assess health status and needs • Interact with members and clients by telephone or in person • Meet members/clients in homes, worksites, or physician offices as needed • Travel up to 40% of the work week

🎯 Requirements

• Must reside in the state of Illinois • Must possess reliable transportation and be willing and able to travel up to 40% of the time from home location • Minimum 3–5 years clinical practical experience • Confidence working at home/independent thinker, using tools to collaborate and connect with teams virtually • Excellent analytical and problem-solving skills • Effective communications, organizational, and interpersonal skills • Ability to work independently • Effective computer skills, including navigating multiple systems and keyboarding • Proficiency with MS Word, Excel, Outlook, PowerPoint, and proprietary applications • Associate's degree required • Active and unencumbered Registered Nurse license in the state of Illinois • Direct/hardwired internet connection to a modem/router within 7 feet of computer • Minimum internet speed of 25 Mbps download and 3 Mbps upload • Quiet, secure, private designated home work location compliant with CVS Health and HIPAA guidelines • Must work within the state and city where currently living • Must provide workspace furniture • Must obtain required internet speeds and adhere to Work From Home requirements • Comfortable setting up and using multiple monitors and navigating multiple applications • Ability to communicate via email, calendar invites, Teams messaging, and virtual meetings • Experience with Microsoft Office 365 or similar Google Workspace applications • Ability to log in to secure systems such as VPN and EHR portals • Ability to follow computer security practices, including strong passwords and recognizing suspicious emails or links • Ability to resolve common technical issues independently • Openness to learning new skills as the workplace evolves
