📋 Description

• Conduct comprehensive assessments of members’ medical, behavioral health, and social determinants of health needs • Spend 50–75% of the day engaging telephonically with members and coordinating their care • Compile clinical information and develop individualized care plans with members • Provide evidence-based disease management education and support • Involve appropriate members of the interdisciplinary care team • Coordinate care to support a seamless healthcare experience • Document care management activities accurately and timely in members’ electronic health records • Collaborate with the Interdisciplinary Care Team to address barriers to care and maintain stable health conditions • Connect members with health plan benefits, community resources, and support services • Meet regulatory requirements within specified timelines • Support Care Team members through clinical decision-making and guidance • Participate in special projects, process improvement initiatives, and mentoring new team members as assigned • Educate and guide members and families on chronic condition management and healthcare navigation • Advocate for necessary services and monitor member progress

🎯 Requirements

• Active and unrestricted Registered Nurse (RN) licensure in New York, or compact licensure in state of residence • Associate’s of Science in Nursing (ASN) degree and relevant experience in a health care-related field (REQUIRED) • Proficient in Microsoft Office Suite, including Word, Excel, Outlook, OneNote, and Teams • Access to a private, dedicated workspace at home • Ability to work independently from home and collaborate virtually • Minimum 3+ years of nursing experience • Minimum 2+ years of case management, discharge planning and/or home healthcare coordination experience • Ability to meet performance and productivity metrics, including call volume, member engagement, and regulatory requirements • Experience or willingness to learn care management within Medicare and Medicaid managed care • Familiarity with community resources and services • Ability to use healthcare technology tools • Effective verbal and written communication • Excellent customer service and engagement skills • BSN preferred • Preferred experience with Medicare and/or Medicaid members, SDoH needs, chronic medical conditions, behavioral health, health-related assessments, and care planning • Bilingual skills, especially English-Spanish, preferred
