📋 Description

• Use critical thinking skills and respond quickly and professionally under stressful and emergent situations • Provide clinical assessment and nursing triage to diverse patients across the health continuum • Provide health education to culturally diverse populations • Direct patients to the most appropriate level of care • Provide telephonic health information and nursing education • Empower patients to make better healthcare decisions • Help patients participate in external programs and available resources • Collaborate with Telehealth providers by referring appropriate patients for virtual care visits • Provide outbound follow-up calls as necessary • Perform other duties as assigned

🎯 Requirements

• Nursing Degree (Diploma, ADN, BSN) required • Active, unrestricted RN licensure in the state where the candidate resides • Ability to obtain multi-state licensure • eNLC preferred or ability to obtain • Two to four years of clinical experience required • Hospital experience in medical-surgical, critical care, or inpatient care setting preferred • Experience in Emergency Room, EMT, or triage • Excellent verbal and written communication skills • Knowledge of health insurance, community resources, and community-based healthcare coordination • Excellent organizational and prioritization skills • Proficiency with computers, software applications, and phone systems • Knowledge of hospital electronic medical records (EMRs) • Manage patient information according to HIPPA guidelines • Ability to plan, supervise, and review the work of clinical and non-clinical staff • Ability to work under stressful conditions or irregular hours
