• Onboarding and orientation for newly hired RN Care Managers • Develop and maintain standardized onboarding curricula, training materials, job aids, and learning resources • Facilitate virtual and in-person training sessions on RN-CM workflows, care management principles, documentation standards, and technology platforms • Coordinate and oversee shadowing experiences and structured learning plans • Monitor onboarding progress and provide regular feedback to new hires and leaders • Validate readiness for independent practice through competency assessments and skills demonstrations • Develop and maintain RN-CM core competency standards • Conduct annual competency assessments and skills validations • Identify knowledge gaps and create targeted development plans • Support remediation plans for RN-CMs requiring additional coaching or performance support • Ensure consistency in practice standards across markets and regions • Serve as a subject matter expert for RN-CM workflows, care coordination, chronic disease management, care planning, transitions of care, and patient engagement • Monitor onboarding and competency outcomes to identify improvement opportunities • Partner with operational leaders to improve quality metrics, care management outcomes, and documentation accuracy
• Active Registered Nurse (RN) license in good standing • Bachelor’s degree in Nursing (BSN) • Minimum 3 years of clinical nursing experience • Minimum 2 years of care management, case management, population health, or chronic disease management experience • Demonstrated excellence as an RN Care Manager • Strong presentation, facilitation, coaching, and communication skills • Experience training, mentoring, or precepting staff • Proficiency with electronic medical records and care management platforms
