📋 Description

• Provide telephonic patient contact and care • Collaborate with health care professionals to provide a meaningful patient experience • Use critical thinking to develop, implement and evaluate comprehensive plans of care for patients with multiple disease states • Make outbound calls to provide education and support for new medication • Support defined patient populations through telecommunications nursing outreach programs • Educate patients on medication adherence and disease states • Provide disease state training, assessment and patient education • Deliver customer service to patients and caregivers while working from home

🎯 Requirements

• Must hold an unrestricted Registered Nurse license in the state of residence • Must have multi-state/compact privileges and ability to be licensed in all non-compact states, territories and the District of Columbia based on business needs • Must meet specific state nursing board requirements • Candidate must be based in Texas or Arizona for this requisition • 3+ years of clinical RN experience • Experience using Microsoft Office, including Word, Excel and Outlook • Previous telephonic nursing experience preferred • EPIC systems experience preferred • Licensure in multiple states preferred • Bachelor’s degree preferred • RN Diploma, Associate’s or Bachelor’s in Nursing
