📋 Description

• Deliver holistic, evidence-based patient care through a remote technology platform • Assess, diagnose, treat, and manage health problems • Provide health counseling and disposition planning for patients aged 18 months and above • Provide education and treatment for pregnancy prevention, STI prevention, and safer sex practices • Evaluate primary care, acute, chronically ill, and transitional care patients • Provide contraceptive care counseling and medication management • Educate patients on health maintenance and respond to patient care inquiries • Document all patient care in the EHR according to MinuteClinic policies and procedures • Coordinate patient care with internal and external colleagues and the patient-centered medical home • Work within a patient care team including Providers, Collaborative Physicians, paraprofessionals, Pharmacists, and other healthcare team members • Work independently, prioritize and solve problems, take initiative, and advocate for patients and the practice • Work the full-time overnight shift from 9:30p to 6a

🎯 Requirements

• Current license in one of: AK, AZ, CA, HI, IL, MN, MO, MT, NM, OR, UT, WA • Minimum of two years of medically relevant experience or equivalent • Completion of a Master’s Degree level Family Nurse Practitioner program • Current National Board Certification • State of Employment license to practice in the Advanced Practice Nurse role • Effective verbal, written, and electronic communication skills • Outstanding organizational skills and ability to multitask • Initiative, problem-solving ability, adaptability, and flexibility • Ability to work without direct supervision and practice autonomously • Proficiency with information management and technology • Capacity to collaborate with professional colleagues to provide quality care • Depending on the market, proficiency in speaking and writing additional languages, including Spanish, may be required • Broadband connectivity • Quiet setting with a neutral background for conducting visits • Ability to uphold patient privacy per CVS Health guidelines • Ability to view and read a computer screen and other electronic devices • Must be willing and able to obtain additional licensure if hired
