📋 Description

• Conduct approximately 30-minute online Video Visits with Medicare Advantage, Medicaid, and certain commercial plan members • Review members’ medications and medical histories • Educate members about their health and answer questions • Work from a private space with adequate internet connection • Maintain a strong visit completion rate and meet monthly capacity and visit targets • Attend regular training, manager, team, and clinical meetings • Maintain training, affiliation, and compliance with Video Visit requirements • Track and meet operational, clinical, and member experience KPIs and metrics • Provide telehealth services only • Interact with health plan members remotely using computer, phone, iPad, and other electronic devices • Practice autonomously without direct supervision

🎯 Requirements

• Board-certified nurse practitioner or physician assistant • Active state licenses in coverage areas • Primary coverage state is Washington • Able to obtain and maintain multi-state licenses in coverage area, as required • Maintain board certification • Provide 40 hours of availability weekly • Consistently provide 320+ capacity units per month • Consistently complete 220+ visits per month • Training, affiliation, and compliance with Video Visit requirements • Experience with Signify Health In-Home Health Evaluations, Video Visits, in-home visits, telehealth visit delivery, or similar format preferred • Broadband connectivity with video and audio enabled • Private, quiet setting with a neutral background • Ability to uphold health plan member privacy per CVS Health guidelines • Effective verbal, written, and electronic communication skills • Ability to work without direct supervision and practice autonomously • Proficiency with information management and technology • FNP or AGNP certification preferred for nurse practitioners
