📋 Description

• The goal of this role is to ensure DaVita is the provider of choice to hospitals. • This teammate will engage renal discharge planners and case managers in an effort to build and strengthen partnerships. • Their focus is to ensure a smooth and seamless experience in support of newly diagnosed and hospitalized dialysis patients to ensure a successful transition to the outpatient setting. • Analyze data insights to highlight its importance and drive growth. • Demonstrate executive presence when delivering insights to upper leadership, ensuring clear and impactful communication. • Leverage a strategic mindset to perform root cause analysis, identifying opportunities based on patient feedback. • Consistently outreach to discharge planners to provide education and build awareness of offerings. • Prioritize key insights and opportunities to align with business goals and stakeholder needs. • Identify opportunities to grow as provider of choice through relationship building and addressing feedback. • Partner with internal field teammates to optimize the admission process and implement best demonstrated practices. • Assist with necessary communication between hospital, admission and clinic teammates to work through patient placements with extenuating circumstances. • Support a smooth and seamless discharge experience for patients and hospital staff to DaVita centers. • Develop comprehensive knowledge of local areas.

🎯 Requirements

• Bachelor’s Degree preferred • Ability to travel to various locations to meet with partners (internal and external) on an as needed basis. 30-40% travel • Includes valid driver’s license in state of residence and insurable driving record • Intermediate computer skills with a variety of software packages including but not limited to Microsoft Word, Excel, PowerPoint, billing systems.
