📋 Description

• Execute tactical, analytical, and strategic work to ensure the model of care team meets clinical and performance goals • Help shape the future of integrated care at DaVita • Provide strategic direction and tactical oversight for model of care initiatives • Drive program goals, including broadening reach and improving clinical impact • Coordinate program team and program initiative plans • Develop strong relationships with internal and external partners • Monitor and manage model of care program performance

• Experience, qualifications, skills, and competencies sufficient to perform the role • Ability to execute tactical, analytical, and strategic work • Ability to provide strategic direction and tactical oversight for model of care initiatives • Ability to coordinate program team and initiative plans • Ability to develop strong relationships with internal and external partners • Ability to monitor and manage program performance
