📋 Description

• Function autonomously and collaborate with healthcare team members to coordinate and facilitate quality, cost-effective care • Minimize fragmentation of the healthcare delivery system for ESRD and CKD patients • Coordinate care among patients, families, physicians, providers, care teams, the community, and the Regional Operations Manager • Provide telephonic, field-based, or combined telephonic and field-based case management • Conduct patient outreach nationally • Help prevent complications, reduce avoidable hospitalizations, and improve overall health for patients with kidney disease

🎯 Requirements

• A.D.N degree from accredited school of nursing required; B.S.N preferred • Current Registered Nurse (RN) license in the state of practice required • Minimum of 1 year of IKC RN experience required • Minimum of three (3) years’ experience in clinical nursing required • Minimum of one (1) years’ experience in renal nursing or case management preferred • Continuing education credits maintained as required by state of practice required • Current CPR certification required • Ability to work a Monday-Friday schedule • Ability to work autonomously • Ability to adjust work hours as needed to accommodate patient outreach • Demonstrated knowledge of data and managing to clinical, financial, and patient satisfaction outcomes • Demonstrated experience and effectiveness in change agent role • Demonstrated knowledge of CQI techniques • Ability to adapt to new/electronic processes and increased productivity expectations • Basic computer skills and proficiency in MS Word and Outlook required • Functional proficiency with DaVita-specific clinical software programs, including Capella, required within 90 days of employment • Home office with internet connectivity of at least 1MB upload and 1MB download speed required
