📋 Description

• Manage and support integrated care for patients with late-stage chronic kidney disease and end stage renal disease • Lead clinical support activities within DaVita Integrated Kidney Care • Contribute to improving patient quality of life, clinical outcomes, and total cost of care • Support the development of DaVita IKC’s model of care and team • Help achieve the vision of making integrated care the standard of care for renal patients • Collaborate with nurses, care coordinators, nurse practitioners, and business professionals • Participate in a compassionate, quality-focused healthcare community

🎯 Requirements

• Experience and qualifications sufficient for a senior manager role in clinical support • Ability to support quality patient care for chronic kidney disease and end stage renal disease populations • Commitment to DaVita IKC’s clinical outcomes, training, growth, and quality objectives
