📋 Description

• Deliver virtual telehealth acute care to members with emergent or critical illness • Evaluate and diagnose acute illnesses • Order and interpret diagnostic tests • Establish care plans and prescribe appropriate medications • Assess quality-of-care interventions, social needs, home health needs, and DME needs • Coordinate care with PCPs, specialists, pharmacy, clinical nursing, social work, family members, and caregivers • Use Devoted's electronic health information system for visits and provide feedback to improve the interface • Provide weekend on-call support to clinical nurses in certain geographies • Support new clinical and organizational programs and initiatives • Integrate innovative virtual care practices • Collaborate with physicians, APPs, medical assistants, documentation experts, practice administrators, social workers, and clinical nurse partners

🎯 Requirements

• At least three years or more working as an APP, preferably in urgent care, emergency medicine, primary care, and/or internal medicine • Role licensure and certification in good standing is required • Ability to get licensed in requested states within 90 days of hire date • Willingness to get licensed in additional states as needed • RN and APRN licenses active and in good standing, or Physician Assistant licenses active and in good standing • Active BLS certification • Willingness to work four 10-hour shifts per week, including one weekend shift per month • Working hours from 11am-9pm EST • Adaptable, eager to learn, and comfortable working in a fast-paced, dynamic setting • Flexibility in scope of practice and willingness to embrace new challenges • Experience in a setting focused on acute or urgent care is desired • Bilingual Spanish/English is a plus • Experience performing visits over telehealth video platforms is desired • Experience managing acute/chronic disease exacerbations is desired
