📋 Description

• Provide diagnostic patient care primarily through virtual consultation by video conference or telephone • Serve as the dispositional authority for after-hours and weekend clinical calls • Assess, diagnose, treat, and document patients’ physical and psychosocial health status • Perform and document physical evaluations and patient histories • Analyze patient trends and develop, document, and implement management plans • Provide health education, physician referrals, case management referrals, and patient/family counseling • Collect and analyze comprehensive and focused patient and family health data • Confer with PCPs and other medical providers regarding health data and guideline compliance • Use telemedicine to support optimal patient outcomes and collaborate with on-call PCPs • Enhance telehealth practices and systems using program-effectiveness data • Communicate through varied formats, tools, and technologies to deliver care • Participate in clinical rounds, conferences, quality and risk management programs, and relevant meetings • Participate in telehealth policy, procedure, and protocol development • Initiate and participate in quality-improvement activities • Coordinate services with other programs and provide feedback to improve patient care • Perform other duties as assigned

🎯 Requirements

• Bachelor’s degree in Nursing (BSN) and graduate of a school of nursing for Advanced Practice Nursing with certification in area of specialty required • Master's degree in Nursing required • Board certification by AANP or ANCC required • Basic Life Support (BLS) certification from the American Heart Association or American Red Cross required upon hire • Compact multistate Nursing license required • Multistate APRN licensure including Florida and at least two licenses among VA, GA, MI, MO, OH, PA, TN, TX, IL, KY, and LA required • Minimum 3 years' acute/primary/urgent care clinical work experience as a Nurse Practitioner required • Minimum 2 years' telehealth work experience preferred • Expert knowledge of medical practices and core job-related functions • Technical capability to conduct telemedicine visits in accordance with state and federal regulations • Proficient in Microsoft Office Suite, including Excel, Word, PowerPoint, and Outlook • Ability and willingness to travel up to 10% for meetings and training • Ability and willingness to travel to Georgia or other states requiring onsite observation and medical-record review • Availability to work evenings/overnights and weekends, depending on assigned schedule • Minimum requirement to work four holidays per calendar year • Spoken and written fluency in English • Ability and willingness to obtain independent/autonomous practice as an NP in applicable states
