📋 Description

• Provide high-touch telehealth visits and facilitate virtual patient onboarding • Identify care gaps and navigate members through the healthcare system • Coordinate across primary care offices and specialty networks • Manage electronic prescription requests and pharmacy communications • Drive referral workflows and track DME, home health care, imaging, and laboratory orders through completion • Collaborate with home health agencies on Start of Care dates and skilled nursing orders • Track lab and diagnostic imaging results and documents for timely clinician review • Manage inbound and outbound referral workflows, including specialist selection and records or authorization collection • Assist with prescription refill requests, medication clarification routing, and adverse event or clinical inquiry escalation under provider guidance • Coordinate medical record collection, information gathering, appointments, and outbound communications with Devoted Medical providers • Review member panels and disease registries to identify and close preventive care gaps • Educate members about preventive screenings and coordinate testing kits, mobile diagnostics, or clinic appointments • Serve as a member advocate and point of contact through calls, outreach queues, and digital messaging portals • Explain medical instructions, plan benefits, and healthcare navigation to members and families • Review internal referrals, conduct welcome outreach, and distribute educational materials and care kits • Perform all delegated clinical tasks under licensed professional supervision; no clinical decision-making or licensure-required activities

🎯 Requirements

• MA Certification is highly preferred; expired MA certifications are acceptable • A copy of an active or expired CMA, RMA, or CCMA certification will be required if applicable • If uncertified, significant MA experience/background may qualify; MA school diploma/transcripts may be requested • Two (2) years of patient-facing healthcare experience • Experience in virtual care, outpatient ambulatory clinics, or telehealth settings strongly preferred • Experience utilizing EHR/EMR systems such as Epic, AthenaHealth, eClinicalWorks, or Cerner • Thorough understanding of HIPAA regulations, patient privacy standards, and clinical documentation compliance • Care coordination and quality metric improvement experience preferred • Experience assisting Medicare Advantage populations with STARS/HEDIS preventive care gaps preferred • Fully bilingual in English and Spanish preferred • Ability to prioritize and manage a high-volume member panel across multiple channels • Prior experience with DME authorization tracking, home health coordination, lab/imaging requisition follow-through, and specialist referral routing • Ability to work Monday–Friday, 9:00 a.m.–5:30 p.m. ET • Weekend availability based on business needs, operational priorities, or member demand
