📋 Description

• Conduct primarily telehealth video visits, with a small volume of home-based visits requiring travel to members' homes • Perform comprehensive assessment visits, including diagnosis and disease review, medication review, STARS/HEDIS quality-of-care interventions, and assessment of social, home health, and DME needs • Perform diagnostic tests, including mobile screenings and fingerstick tests • Coordinate care with members' PCPs, specialists, pharmacy, clinical nursing, social work, family members, and caregivers • Use Devoted Medical's electronic health information system and provide feedback to improve the interface • Provide telehealth visits to members in other geographies when appropriate and compliant with licensure • Participate in infrequent weekend on-call coverage in certain geographies to support clinical nurses • Work closely with physicians, APRNs, medical assistants, documentation experts, practice administrators, and health-plan partners • Deliver comprehensive, collaborative, member-focused care through virtual and home-based visits

🎯 Requirements

• APRN with 3 or more years working in outpatient clinical practice • Experience in primary care, family medicine, geriatrics and/or palliative care • Experience performing Medicare annual wellness visits or in-home comprehensive visits with elderly patients or Medicare patients • Experience performing visits over telehealth video platforms • Understanding of managed care, including appropriately assessing STARS/HEDIS measures, coding clinical comorbidities, and identifying clinical care gaps • Strong desire to continue practicing clinical nursing and perform house calls as well as virtual visits • Active and clear RN and APRN license in the state of [MARKET] required at time of hire and maintained while employed • APRN certification required at time of hire and maintained while employed • Active BLS required at time of hire and maintained while employed • Multi-state licensure preferred or willingness to obtain additional licensure as requested • Extra interest in bilingual Spanish or Haitian Creole speakers or strong cultural competencies across a range of cultures
