📋 Description

• Manage and navigate Medicaid and Medicare benefits and eligibility for clients • Understand and work with different HUSKY plan options and requirements • Collaborate with client case managers, conservators, and other care team members regarding benefits and coverage • Partner with the Department of Social Services (DSS) and navigate state systems to resolve client benefit issues • Manage the Medicaid renewal process, including required documentation and follow-up • Assist clients with accessing and maintaining other state benefits and resources • Identify gaps or interruptions in coverage and work toward reinstatement of Medicaid/Medicare benefits • Maintain accurate documentation and follow up with appropriate agencies and stakeholders to ensure benefits remain active • Perform activities aligned with the vision of Elara Caring's board of directors, executive team, and Home Health leadership • Report to the Clinical Team Manager • Provide social work support that helps keep clients at home

🎯 Requirements

• Degree of Social Work from a school of Social Work accredited by the Council of Social Work Education • 1+ year of social work experience in a healthcare setting • Current State License as a Social Worker (preferred) • Excellent verbal and written communication skills • Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment • Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations • Strong knowledge of Connecticut Medicaid and Medicare • Knowledge of different HUSKY insurance plans • Knowledge of renewal processes and state benefits • Experience with redeterminations, spend down, Title 19 applications, reinstatement, and navigating the DSS system
