• Guide patients and families through chronic kidney disease, end-stage kidney disease, and end-of-life stages • Drive Goals of Care discussions • Facilitate Advance Care Planning • Provide anticipatory grief support • Address behavioral health concerns • Provide virtual patient support remotely • Conduct palliative social work assessments and behavioral health interventions • Support patients with advanced kidney disease and caregivers • Educate patients, families, and caregivers about community resources and palliative and hospice services • Advocate for patient needs and preferences • Collaborate with interdisciplinary teams to develop and implement individualized care plans • Liaise with dialysis clinic staff, nephrology practices, and outside providers to deliver goal-concordant care
• Current, active LCSW licensure in California • Residence in Pacific Standard Time (PST) or Mountain Standard Time (MST) • Master’s degree in Social Work from an accredited institution • One year of hospice or palliative care experience • At least 6–8 years of social work experience, preferably in healthcare settings • Strong interpersonal and communication skills • Ability to provide quality patient care in a remote work setting • Thorough assessment skills • Familiarity with palliative care principles • Knowledge of grief counseling techniques • Knowledge of advance care planning processes • Knowledge of behavioral health interventions • Empathetic, self-directed, and adaptable approach • Ability to handle emotional situations with professionalism
