📋 Description

• Assess patients’ physical, behavioral, psychosocial, and environmental needs • Develop, coordinate, implement, and evaluate individualized plans of care • Manage an assigned caseload and complete initial and ongoing nursing assessments • Monitor patients for changes in condition, safety concerns, and barriers to treatment • Communicate updates to physicians, clinical leadership, caregivers, and other care team members • Educate patients and caregivers about medications, symptom management, safety, and treatment goals • Update plans of care based on patients’ changing needs and progress • Coordinate services with an interdisciplinary team to support positive patient outcomes • Supervise and document care provided by LPNs and behavioral health aides when applicable • Maintain accurate and timely clinical documentation

🎯 Requirements

• Current, unrestricted RN license in the state of practice • Associate or bachelor’s degree in nursing • At least one year of clinical nursing experience • Behavioral health, home health, or hospice experience preferred • Homecare HomeBase experience preferred • Dependable vehicle, valid driver’s license, and current automobile insurance • Willingness to travel throughout the assigned territory • Ability to travel up to 50% • Ability to sit, stand, bend, lift and move intermittently • Ability to lift 50–100 lbs.
