📋 Description

• Provide primary and urgent health care to patients by telephone and tele-video • Develop and implement clinical plans of care for adult patients with chronic and complex conditions • Gather patient histories, perform physical examinations, and obtain diagnostics as needed • Develop and implement treatment plans aligned with patient goals and current conditions • Identify and close gaps in care • Address patients’ and families’ physical and psychosocial needs with interdisciplinary team support • Educate patients and families about medications, side effects, illness progression, diet, nutrition, adherence, and crisis prevention • Coordinate care and resources with clinical team members, physicians, and medical providers • Maintain patient medical records and clinical documentation • Participate in required continuing education • Prescribe medication as permitted by state authority • Collaborate with company physicians, other providers, and family members to develop complex plans of care • Provide clinical and non-clinical support to patients receiving home and community-based services

🎯 Requirements

• Bilingual or multi-language skills required • Master of Science in Nursing required • Active national Nurse Practitioner certification required • Valid, current, active, and unrestricted Family or Adult Nurse Practitioner license in New York required • Valid, current, active RN license required • 2+ years of experience managing complex care cases • Experience working with Electronic Medical Records (EMR) • Bilingual Spanish preferred • Medicaid registration preferred • DEA registration or eligibility preferred • Experience managing complex care cases for developmental disabilities and chronically ill patients strongly preferred • Candidates must reside within commuting distance of an office unless an accommodation is granted • Must work Monday–Friday, 8:00 a.m.–5:00 p.m. local time, with on-call coverage of 2–3 nights every 4–6 weeks and holidays • New candidates in certain patient/member-facing roles must be vaccinated against COVID-19 and influenza unless an acceptable explanation is provided
