📋 Description

• Collaborate with company physicians, patients’ other physicians and providers, and family members to develop complex plans of care • Provide clinical and non-clinical support to patients • Provide primary and urgent health care by telephone and tele-video to patients receiving home and community-based services • Develop and implement clinical plans of care for adults with chronic and complex conditions • Gather medical histories, perform physical examinations, and obtain diagnostics as needed • Develop and implement treatment plans based on patients’ goals of care and current conditions • Identify and close gaps in care • Address patients’ and families’ physical and psychosocial needs with interdisciplinary team support • Educate patients and families about medications, side effects, illness progression, diet and nutrition, adherence, and crisis anticipation and prevention • Coordinate care and resources with clinical team members, physicians, and medical providers • Maintain patient medical records and documentation • Participate in required continuing education • Prescribe medication as permitted by state authority • Work Monday–Friday, 8:00 a.m.–5:00 p.m. local time, with an on-call schedule

🎯 Requirements

• Bilingual or multi-language skills required • MS in Nursing required • Active, national NP certification required • Valid, current, active, and unrestricted Family or Adult Nurse Practitioner (NP) license in New York required • Valid, current, active RN license required • 2+ years of experience managing complex care cases • Experience working with Electronic Medical Records (EMR) • Bilingual in Spanish highly preferred • Registered with Medicaid preferred • DEA registration or eligibility preferred • Experience managing complex care cases for developmental disabilities and chronically ill patients strongly preferred • Must reside within a reasonable commuting distance from the posting location(s), unless accommodation is granted as required by law • COVID-19 and Influenza vaccination required for certain patient/member-facing roles, subject to acceptable explanation
