📋 Description

• Coordinate follow-up care plan needs by scheduling appointments or enrolling members in programs • Assess member compliance with medical treatment plans by telephone or through on-site visits • Identify barriers to plan compliance and coordinate resolutions • Identify opportunities affecting quality goals and recommend process improvements • Recommend treatment plan modifications and determine the need for additional services with case management and providers • Coordinate identification of and referral to local, state or federally funded programs • Coach members on ways to reduce health risks • Prepare reports documenting case and compliance updates • Establish and maintain relationships with agencies identified in appropriate contracts • Work 5 eight-hour shifts per week, including one Saturday and one Sunday per month • Work Eastern Time hours of 8:00 AM–5:00 PM or Central Time hours of 7:00 AM–4:00 PM

🎯 Requirements

• H.S. diploma or equivalent • Minimum of 1 year related experience • Any combination of education and experience which would provide an equivalent background • Bilingual or multi-language skills may be required for the Carelon - CareBridge business unit • Certified nurse assistant or certified medical assistant preferred • BS/BA degree in a related field preferred • Bilingual candidates preferred • Strong oral, written and interpersonal communication skills • Problem-solving skills • Facilitation skills • Analytical skills • Must be within a reasonable commuting distance from one of the posting locations unless an accommodation is granted as required by law • New candidates in certain patient/member-facing roles must become vaccinated against COVID-19 and Influenza, unless an acceptable explanation is provided
