📋 Description

• Complete individualized patient care plans and perform care management and care coordination services using Ennoble Care’s electronic medical record system • Contact patients frequently to provide care coordination and support and manage compliance with care management programs • Document all client communications accurately • Communicate with the Primary Care Clinician or designee regarding emergent needs and life-threatening episodes and ensure comprehensive care plans are complete and accurate • Keep the Team Supervisor informed of issues pertinent to the care plan process • Work with cross-organizational areas to meet the needs of patients, family members, and partner facilities • Use technology including secure email, telephone systems, and electronic medical records • Adhere to documentation protocols and best practices for daily work logs, issue escalation, and internal communications • Provide exceptional customer service • Support department team members productively and constructively • Operate computers, telephones, copy machines, and other office equipment as necessary

🎯 Requirements

• Must be a Licensed Practical Nurse (LPN) • Comfortable speaking on the phone for large amounts of the day • Compassionate and empathetic toward patients, with exceptional customer service • Ability to take accurate notes and document each task promptly • Ability to multitask across patients and workstreams while remaining organized and efficient • Ability to thrive in a fast-paced environment • Able to work Saturday and Sunday 8:00am–4:30pm EST and Monday, Tuesday, and Wednesday 9:30am–6:00pm EST • Proficient with computers, Outlook, and other Microsoft Office programs • Knowledge of basic healthcare terms, conditions, roles, and care principles is helpful • Must pass a drug screen and background check • Positive attitude, adaptability, teamwork, and willingness to continuously learn new skills • Must disclose any current or future need for employment-based immigration sponsorship before an offer is extended
