• Develop, assess, and coordinate care management activities based on member needs • Develop or contribute to personalized care plans/service plans • Educate members and families/caregivers on services, benefit options, care options, referrals, healthcare benefits, and provider instructions • Evaluate member needs, barriers to care, and available resources • Identify providers, specialists, and community resources needed for care • Provide psychosocial and resource support for employment, education, housing, food, participant direction, independent living, justice, and foster care resources • Coordinate between members/families/caregivers and care provider teams to ensure timely access to care and services • Monitor progress toward care plan goals and member status or changes in condition • Collaborate with healthcare providers on care plan revisions and refer members for further care management evaluation • Collect, document, and maintain member information and care management activities for regulatory compliance • Perform on-site visits to assess member needs and collaborate with providers or resources, as appropriate • Perform other duties as assigned and comply with all policies and standards
• Bachelor’s degree and 2–4 years of related experience • Graduate from an Accredited School of Nursing if holding clinical licensure • Equivalent experience may substitute through applicable knowledge, duties, scope, and skills reflective of the position level • Current state clinical license preferred • Missouri LPN / LVN license highly preferred • Ability to work Monday–Friday, 8am–5pm Central • Ability to develop, assess, and coordinate care plans and services • Knowledge of healthcare benefits, referrals, providers, specialists, and community resources • Ability to collect and maintain documentation in compliance with state, federal, and third-party payer regulations
